Use of Endoscopes in Nose Surgery has revolutionised the field of ENT. We can reach SKULL BASE through Nose with Endoscopes.
Presenting a case of 55yr old female with spontaneous CSF leak(brain fluid) through lt Nose for 1yr.
CT & MRI Cysternography was done and leak site was found to be lateral lamella of left cribriform plate.
After doing Endoscopic dissection, site of leak is detected and confirmed by applying Flourescine strip which turned yellowish green after coming in contact with CSF FLUID(BRAIN FLUID)-Showing in image.
Six layer protection was given;
1st layer: Fat put over leak site.
2nd layer: Tissue glue spread over fat.
3rd layer: Fascia Lata spread over glue.
4th layer: Tissue glue over Fascia Lata.
5th layer: Abgel.
6th layer: Tissue glue over Abgel.
Leak completely stopped after 6layer protection.
Patient was discharged after a week.
Follow up was done after a month with complete healing of six layer protection.
